A D4 Medical is required:

  • For a provisional driving licence (first application)

  • Every year from the age of 65

Taxi (Hackney Carriage / Private Hire) Medical

Hackney carriage or private hire (Taxi) drivers must obtain and submit a Medical Questionnaire completed by a medical practitioner — usually every 3 years. Your local authority (council) will provide you with the medical form, or you can find it on their website. It will be completed by the doctor.

Disclaimer: Prior to booking, you will need to check with your local authority (council) that any registered doctor can perform the medical — not just your own GP. Once booked, medicals cannot be cancelled.
